This award will recognise a woman with a learning disability being bold and passionate about changing their community.
There are two ways to nominate for the Mencap Community Connector Award. If you need any more information or support with your nomination, please contact The Wales Learning Disability Helpline on 0808 8000 300.

These are the questions that we will need you to answer either on the form or on the video.
- Please tell us about the person you are nominating (this could be you)
- What has the nominee achieved? What difference have they made to your life or the lives of others?
- What barriers or challenges have they overcome?
- How have they encouraged and supported women?
